Acknowledge participation in the activities at a Jungle George’s (the company) facility, the undersigned, on his or her own behalf, and on behalf of any executors, heirs, successors and assigns, hereby acknowledges and agrees as follows:

The undersigned knowingly and fully assumes all risks, known and unknown, of anyone associated with them while participating in activities at Jungle George’s facility including but not limited to birthday parties, family events, general play, field trips, church or other youth group events, and waives all claims for damage to personal property or injury to person arising from such participation. This release and waiver covers risks of death, serious injury, and property loss whether arising from (a) negligence or carelessness on the part of the persons or entities being released and other participants, or (b) dangerous or defective equipment.

The undersigned certifies that he or she is physically fit and may participate in the activities available at a Jungle George’s facility, and he’s not been advised to the contrary by a qualified medical professional.

The undersigned on his or her behalf and on behalf of any executors, heirs, successors and assigns, hereby releases, discharges, and holds harmless any authorized entity doing business as Jungle George’s, including its officers, agents, employees, representatives, and all affiliates, from any and all claims, damages, or liability arising from death, disability, personal injury, property damage, or theft or any kind of action.

The undersigned person does hereby represent that they, in truth, acting in such capacity and agrees to save and hold harmless and indemnify each and all the parties affiliated to you of all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act, and releases said parties on behalf of the person and the guardian of such person.

The undersigned agrees to abide with the rules and conditions of participation posted at or explained by an employee at Jungle George’s facility. The undersigned is knowingly and fully giving permission to receive or have received any text messages and phone calls from Jungle George’s and marketing representatives. If the undersigned experiences any significant hazard or injury during each participation, the undersigned will promptly inform an employee of the company facility which they are currently attending.

The undersigned acknowledges that this release and waiver of liability form will be used and relied upon by the company and that it will govern the undersigned’s actions and rights. Waiver will always remain valid and never expire.

By clicking and submitting a digital waiver, you are taking on all risk and responsibility upon yourself and any person or property affiliated with you. Jungle Georges does not take responsibility of supervising your persons or property. You’re entering a free indoor public playground. Please be respectful to the facility, staff and fellow visitors.
